Client Forms

Veterinary Supervision Form: tracydulockmassage.massagetherapy.com/files/vetsupervisionTD.pdf

  • All Texas canine massage clients must provide this form before the first appointment;
  • This form with Tracy's name, contact information, etc. must be used - she cannot legally accept the form of another practitioner;
  • Practitioners who do not maintain the forms are subject to a large fine/sanctions.
  • NOTE:  Feel free to scan and email completed forms to me anytime, even if you don't plan on scheduling in the near future. I keep all forms file against the day your dog might need me.  Most vet offices have scanners if you don't; my email is on the form.

Tracy appreciates your help keeping her in compliance with Texas law.

These services are not a substitute for medical or veterinary care.

If you have any health concerns regarding your animal, you should consult a licensed veterinarian as soon as possible.  Per Texas law, therapeutic massage on an animal is required to be done under the direct or general supervision of a licensed veterinarian.  Since Tracy is not a veterinarian and is acting as an independent contractor, a veterinary supervision form is required prior to your first visit in order to be in compliance with Texas Administrative Code §573.14.
